Abstract: 

Today, hepatocellular carcinoma is one of the most common and dangerous types of liver cancer. In most treating methods of liver cancer such as using microwaves, radio frequency radiation, etc., the electrode enters the patient's body, so these methods are ineffective in the case of metastatic cancer. High-intensity focused ultrasound (HIFU) therapy is an appropriate non-invasive treatment method which no external elements enter the patient's body and can be used frequently. However, the success of the treatment depends on treatment parameters such as exposure time. Therefore, in this paper HIFU heat therapy is simulated for the treatment of hepatocellular carcinoma as a non-invasive method. For this purpose, Model-JC HIFU System, tissue and tumor, and water are included in the simulations. The tumor is considered as a symmetrical sphere with a radius of 0. 4 cm. Then Helmholtz and bioheat equations are solved by finite element method (FEM) with considering the boundary and initial conditions. The frequency and power of ultrasonic waves are 1 MHz and 150 W, respectively. Investigation of the temperature distribution and fraction of necrotic tissue declare that the exposure time of HIFU is an important parameter. If the treatment is performed in 100 seconds with this HIFU system, the oval area with large diameter and small diameter of 21. 1 and 9. 95 mm is completely destroyed. Also, complete tumor destruction is available in HIFU exposure time of 72 seconds. Furthermore, the results also show that this method is suitable for deep and oval tumors.

Abstract: 

Pre-exposure to hyperoxic gas (95%) has been shown to protect the heart and central nervous system from ischemia-reperfusion injury. In the present study we investigated whether oxygen pretreatment induces delayed renal protection in rats. The possible role of some renal antioxidant agents was also investigated. Adult male Wistar rats were kept in a hyperoxic (HO) (95% O2) environment for 0.5h, 1h, 2h, 3h, 6h, 2h/day for 3 consecutive days and 4h/day for 6 consecutive days and control group (IR) animals were kept in the cage with no HO; one day before subjecting their kidney to 40 minutes of ischemia and 24h of reperfusion. Renal function was assessed by comparing plasma creatinine (Cr), blood urea nitrogen (BUN), creatinine clearance (CLCr) and fractional excretion of sodium (FENa%). Histopathological injury score was also determined according to Jablonski method. To examine the antioxidant system induction by hyperoxia we measured renal catalase and superoxide dismutase activity and renal glutathione and malondialdehyde content. Our data demonstrated that only in 4h/day HO for 6 consecutive days the renal function tests (Cr, CLCr, BUN and FENa%) and Jablonski histological injury were better than control group (p<0.05). The beneficial effect of oxygen pretreatment in this group was associated with increased renal catalase activity compared with those obtained from control group (p<0.05). These findings suggest that repeated exposure to hyperoxic (95% O2) environment can reduce subsequent rat's renal ischemia-reperfusion damage. Induction of endogenous antioxidant system may partially explain this beneficial effect of hyperoxic preconditioning.

Abstract: 

Background & Aim: In survival studies, incidence of competing risks causes that the time of event of interest to be unknown. Analysis of competing risk data, often implemented using hazard-based method under proportional hazard assumption. In this study, we interpreted covariate effect under accelerated failure time model and cause-specific survival function. Methods & Materials: We considered Weibull hazard and survival function as causespecific hazard and survival function and explored the relation between these function. Estimation of parameters performed using Bayesian methods with non-informative priors that implemented in R2WinBUGS package of R software. Results: Simulation study showed that, the relation between hazard and survival parameters for Weibull distribution is also established between parameters of cause-specific hazard and cause-specific survival function. This relation also verified in PBC data set for logarithm of serum bilirubin and D-penicillamine effect. Conclusion: Although in competing risk studies, most of the analysis performed under PH assumption, analysis based on AFT models will also be applicable for these data. In these setting, coefficients can be interpreted as effects of covariate on time to each event.

Abstract: 

Animal bites are a frequent reason for primary care and emergency department visits by children. Rabies is endemic in Iran, with 5 to 11 deaths reported annually. Over 90% of rabies cases in the country are caused by dog bites, and more than 50% of the victims are children under 15 years old. The mortality rate of rabies is 100%. A 4-year-old girl, who had been bitten by a dog and sustained facial lacerations 16 days prior to admission, presented with fever and loss of consciousness. Despite wound washing, receiving four doses of the rabies vaccine and immunoglobulin, and taking cephalexin on the fifth day after the bite, she developed rabies encephalitis. Rabies was confirmed through a skin biopsy and saliva PCR. Treatment included vancomycin, meropenem, and acyclovir in addition to favipiravir. The patient was intubated due to impaired consciousness and respiratory issues, and her hypertension was managed with medication. A brain CT scan showed normal findings. Cerebrospinal fluid analysis revealed protein at 52, glucose at 63, and no cellular presence. Vancomycin and acyclovir were discontinued when blood culture, wound culture, cerebrospinal fluid, and PCR results returned negative. Echocardiography results were acceptable, and the patient’s lab tests showed no other abnormalities except for leukocytosis at admission, an ESR of 60, and a CRP of 40. The patient remained intubated for two weeks and continued to receive supportive measures, ongoing drug therapy, ventilator adjustments, and water and electrolyte balance management. During hospitalization, she showed no response to the apnea test, oculovestibular reflex, or doll's eye maneuver. Two weeks later, she developed bradycardia and subsequently died, with a final diagnosis of furious rabies. Despite receiving the rabies vaccine and immunoglobulin, the patient died within a month of the bite, which may be attributed to the location of the bite on her face, the timing of her treatment, or the quality of the vaccine.

Abstract: 

Introduction In radiography, dose and image quality are dependent on radiographic parameters. The problem is caused from incorrect use of radiography equipment and from the radiation exposure to patients much more than required. Therefore, the aim of this study was to implement a quality-control program to detect changes in exposure parameters, which may affect diagnosis or patient radiation dose. Materials and Methods This cross-sectional study was performed on seven stationary X-ray units in six hospitals of Lorestan province. The measurements were performed, using a factory-calibrated Barracuda dosimeter (model: SE-43137). Results According to the results, the highest output was obtained in A Hospital (M1 device), ranging from 107×10-3 to 147×10-3 mGy/mAs. The evaluation of tube voltage accuracy showed a deviation from the standard value, which ranged between 0. 81% (M1 device) and 17. 94% (M2 device) at A Hospital. The deviation ranges at other hospitals were as follows: 0. 30-27. 52% in B Hospital (the highest in this study), 8. 11-20. 34% in C Hospital, 1. 68-2. 58% in D Hospital, 0. 90-2. 42% in E Hospital and 0. 10-1. 63% in F Hospital. The evaluation of exposure time accuracy showed that E, C, D and A (M2 device) hospitals complied with the requirements (allowing a deviation of ± 5%), whereas A (M1 device), F and B hospitals exceeded the permitted limit. Conclusion The results of this study showed that old X-ray equipments with poor or no maintenance are probably the main sources of reducing radiographic image quality and increasing patient radiation dose.

Abstract: 

Background & Aims: Body Mass Index (BMI) is a factor that affects the quality of panoramic images. This study aimed to evaluate the quality of panoramic digital images with Safe Beam Exposure (SBE) and manual dose adjustment among patients with different BMI levels. Material & Methods: In this cross-sectional study, 200 patients, aged 20-60 years who were prescribed panoramic radiography in the course of their treatment were selected randomly as samples. 50% of the subjects were allocated to the manual dose adjustment group and 50% were distributed randomly to the SBE technology group. After grading quality of the images, scores were entered into the SPSS software. Results: The mean age of participants was 37. 96± 11. 96. 53. 5% of participants were women and 46. 5% were men. 87% of the samples fell in BMI of 18. 5 ≤ to ≥ 30. The overall quality of the images included noise (p=0. 0003), contrast (p=0. 0001), density (p=0. 0002), and resolution (p=0. 0003) and anatomical indicators included Laminadora (p= 0. 0003), edge sharpness of teeth (p = 0. 0002), mandibular canal walls (p = 0. 0001), apex end of upper molar teeth (p = 0. 0002), and DEJ limit (p= 0. 0002). SBE technology in various categories of BMI was better than manual dose adjustment. Conclusion: SBE technology could be used as a new technology to obtain images with higher quality and better anatomical resolution in various categories of BMI. As BMI decreases, the diagnostic quality of the different anatomical landmarks examined in the panoramic images increases.
